The United Hospice Foundation was established in 1998 through the generosity of corporate donors and the grateful patients and families of United Hospice, Inc., the largest hospice provider in Georgia. Over the years, the Foundation has supported end-of-life programs for the general public and health care providers throughout the state.

The Foundation is an independent, non-profit, 501(c)(3) organization that relies on donations and grants to fulfill its mission. The Foundation's home office is located in Norcross, Georgia, where it shares office space and administrative support with UHS-Pruitt Corporation.
